Pentosan GOLD + Halo Injection

VET ONLY PRODUCT - Intravenous Equine Joint Treatment with Pentosan Polysulfate, Glucosamine and Hyaluronic Acid
COMPOSITION

Each 12 mL PENTOSAN GOLD INJECTION vial contains:
1,500 mg Pentosan Polysulfate   (125 mg/mL)
2,400 mg N-Acetyl Glucosamine   (200 mg/mL)

Each 6 mL HALO INJECTION vial contains:
60 mg Sodium Hyaluronate   (10 mg/mL)

Actions

Active ingredients Pentosan Polysulfate, Glucosamine and Hyaluronic Acid are well documented and utilised individually in the treatment of joint disease yet their actions are closely linked.  The logical combination of all 3 products for single intravenous injection aims to enhance the horse's response to treatment.

Pentosan Polysulfate
Pentosan polysulfate (PPS) produces anti-inflammatory, anti-arthritic and chondroprotective effects via 5 main actions:
1. Stimulation of cartilage cells (chondrocytes) to increase the synthesis of proteoglycans;
2. Stimulation of hyaluronic acid (HA) production by synovial cells to increase the volume and viscosity of synovial fluid;
3. Direct anti-inflammatory activity within and around the joint by inhibiting the arachidonic acid cascade;
4. Inhibition of neutrophil migration into joints and their associated release of degradative enzymes;
5. Fibrinolytic (anticoagulant) activity which improves blood circulation to bone and structures around the joint.

Glucosamine
Glucosamine is utilised in the production of glycosaminoglycans (GAGs), including hyaluronic acid (HA), and multiples of these complexes combine to form large proteoglycans in the matrix of cartilage.  As such, glucosamine is a fundamental building block of the cartilage structure, and a growing body of evidence supports the pain-relieving and potentially chondroprotective properties of glucosamine.  N-Acetyl Glucosamine actively stimulates and provides substrate for the secretion of GAGs in articular cartilage, as well as exerting an anti-inflammatory effect and inhibiting degradation of the connective tissues of the joints.1

Sodium Hyaluronate
Sodium Hyaluronate is the sodium salt of the GAG hyaluronic acid (HA) which forms proteoglycans in the matrix of cartilage and provides visco-elastic and lubricating properties to joint fluid.  In joint disease, inflammation breaks down HA which results in lowered viscosity of the joint fluid and further influx of inflammatory cells, exacerbating cartilage damage and joint disease.  Supplementation of HA helps to restore lubrication of the joint, reduce inflammatory infiltrates and minimise ongoing damage.

Field Trials - In Australian trials of Pentosan GOLD + Halo Injection, veterinarians reported "Good" to "Very Good" response in 81% [67 of 83] of the osteoarthritic horses treated.  Positive responses included improved gait, extension, racing performance and attitude.  Also reported were reductions in pain on flexion, stiffness, joint effusion and soreness after racing.

When the responses to Pentosan GOLD + Halo were rated in comparison to the horses' previous treatment with pentosan  polysulfate and/or hyaluronic acid, 86 % [63 of 73] were graded as "Better" or "Much Better".

Intravenous Administration - A multi-dose safety study in horses has demonstrated the safety of Pentosan GOLD + Halo when administered intravenously.  At normal dosage, mild induced elevation of aPTT (activated partial thromboplastin time - measure of coagulation) was returned to baseline levels by 8 hours after each weekly injection during the 6 week trial.2

Dosage and Administration
For adult horses (500 kg) draw up entire contents of both vials (total volume 18 mL) into a single syringe and administer by intravenous injection using a 21 gauge needle (or smaller). 
This dose contains 1,500 mg Pentosan Polysulfate (3 mg/kg) + 2,400 mg N-Acetyl Glucosamine (4.8 mg/kg) + 60 mg Sodium Hyaluronate (0.12 mg/kg).

Storage
REFRIGERATION NOT REQUIRED. Store below 25C (Air conditioning).  Protect from light.
Availability
This product combination is NOT registered in Australia and is only available under APVMA permit PER 11528 to a registered veterinarian for intravenous administration to aid in the treatment of non-infectious inflammatory joint disease in horses under relevant state prescribing laws.  Before use, veterinarians must read the permit supplied as a leaflet with the product.  This permit does not allow the sale of Pentosan GOLD vials individually.

Warnings
The efficacy and safety of this product combination has not been assessed by the APVMA.
Contraindicated in horses with clotting defects, traumatic haemorrhage, infection, liver/kidney failure, or within 2 days of surgery.

Use in combination with an oral joint product such as Cosequin or Joint Guard for best results.
